Reports of breakthrough cases of COVID-19 among the fully vaccinated have many people doubting the vaccines effectiveness. However, the CDC says that these breakthrough cases are not an indication that the vaccines dont work. So far, breakthrough infections have occurred among a small amount of fully vaccinated people, even with the delta variant. These infections have been mild so far as compared to those who were unvaccinated. While there have been some outliers, the CDC reports that fully vaccinated people are much less likely to be hospitalized or die than people with similar risk factors who are not vaccinated. Because this is a constantly evolving situation, they are working with state and local health departments to investigate breakthrough cases of COVID-19.

Reducing the risk of transmitting the coronavirus relies on developing strong immunity against the virus. But immunity, even from the vaccines, fades over time. Scientists are actively monitoring people whove had COVID-19 vaccines to understand how long vaccine immunity is likely to last, and if and when booster shots will be required.

However, none of the COVID vaccines approved for use anywhere around the world so far use live virus.

Instead, they use other technologies to train our bodies to recognise SARS-CoV-2 and to mount a protective immune response should we ever be exposed to it.

For instance, the AstraZeneca vaccine is a viral vector vaccine. This uses a modified chimpanzee virus to carry into the body the genetic instructions to produce the SARS-CoV-2 spike protein. Your body then uses these instructions to make the spike protein, and to raise a protective immune response.

The Pfizer vaccine is an mRNA vaccine, which contains the genetic material to code for the spike protein. Once inside your cells, your body uses those instructions to make spike protein, again raising a protective immune response.

COVID vaccines dont give you the disease or give you a positive COVID test. Again, they dont contain live virus. They contain fragments of spike protein or the instructions on how to make it.

Even if you could shed spike protein after vaccination, that wouldnt be enough to cause an infection. For that you need the entire virus, which the vaccines dont contain.

And the mRNA in the Pfizer and Moderna vaccines is very short-lived, and is quickly degraded in our cells. Again, the mRNA wouldnt be enough to cause an infection. It would need to be packaged inside a live virus, which our vaccines dont contain.

A Cdc Document Gives New Details On Just How Dangerous The Delta Variant Really Is

The CDC said the finding that fully vaccinated people could spread the virus was behind its move to change its mask guidance.

“High viral loads suggest an increased risk of transmission and raised concern that, unlike with other variants, vaccinated people infected with Delta can transmit the virus,” Dr. Rochelle Walensky, the CDC’s director, said in a statement Friday.

“This finding is concerning and was a pivotal discovery leading to CDC’s updated mask recommendation. The masking recommendation was updated to ensure the vaccinated public would not unknowingly transmit virus to others, including their unvaccinated or immunocompromised loved ones,” Walensky said.

What About The New Coronavirus Variants

New variants of coronavirus have emerged in recent months, and recent studies show that vaccines are less effective against certain ones, like the B1351 variant first identified in South Africa.

Every time SARS-CoV-2 replicates, it gets new mutations. In recent months, researchers have found new variants that are more infective meaning a person needs to breathe in less virus to become infected and other variants that are more transmissible – meaning they increase the amount of virus a person sheds. And researchers have also found at least one new variant that seems to be better at evading the immune system, according to early data.

So how does this relate to vaccines and transmission?

For the South Africa variant, vaccines still provide greater than 85% protection from getting severely ill with COVID19. But when you count mild and moderate cases, they provide, at best, only about 50%-60% protection. That means at least 40% of vaccinated people will still have a strong enough infection and enough virus in their body to cause at least moderate disease.

If vaccinated people have more virus in their bodies and it takes less of that virus to infect another person, there will be higher probability a vaccinated person could transmit these new strains of the coronavirus.

The good news is that the vaccines continue to be highly effective against the virus in preventing hospitalization and death from COVID-19. For instance, three studies from Canada, Singapore and Scotland have found that the Pfizer vaccine provides more than 90% protection against hospitalization and death.

Indeed, the Provincetown outbreak demonstrates the vaccines’ effectiveness.

Immunization with the COVID-19 vaccines provides the best protection within two weeks of being fully vaccinated. A person is considered fully vaccinated two weeks after receiving the second dose of Pfizers or Modernas vaccine or one dose of Johnson & Johnsons.;If someone tests positive for COVID-19 or becomes ill a few days later, they most likely were exposed before being fully vaccinated. There are reported cases of illness and/or exposure after the vaccines, but the complications of the disease for those not vaccinated yet has been of greater magnitude.;

Breakthrough Cases And Delta: Background

Until recently, scientists were unsure whether fully vaccinated people who became infected with COVID-19 could transmit it to others. But the CDC report released in late July made it clear that some vaccinated people can get Delta in a breakthrough infectionand may be contagious.; ;

The CDC added that breakthrough infections occur in only a small proportion of vaccinated people and of the breakthrough infections, transmission by the vaccinated appears to only be a small part of overall spread of the virus. What we do know, says Dr. Meyer, is that there is less circulating virus in the community as a result of vaccination. When we look at vaccinations compared to cases on a population levelwe see that as the number of people vaccinated rises, the number of cases decreases, she says. This is likely due to the fact that people who are vaccinated are not becoming infected as often, but also that they are not forward-transmitting the virus as often.

But more research is needed. The CDC says that “studies are underway to understand the level and duration of transmissibility from Delta vaccine breakthrough infections.”

Whats Required For A Vaccine To Be 100% Effective At Preventing Infection

In order to entirely prevent infection, vaccines would need to induce whats called sterilizing immunity, a type of immunity that prevents a pathogenin this case the coronavirusfrom infecting any cells. If the virus cannot infect cells, then the host cannot transmit it to others.

In a best-case scenario, all vaccines would provide sterilizing immunity, meaning they would protect against disease and prevent transmission. But in practice, most vaccines dont do this. The influenza, rotavirus, and pertussis vaccines, among others, can prevent serious illness from developing, but they dont reach the level of sterilizing immunity.

The same is true for the three authorized COVID-19 vaccines. But studies of the real-world effectiveness of the Pfizer-BioNTech and Moderna vaccines suggest that they can substantially reduce the risk of infection.

And reducing infection is tied to reducing transmission. If youre not infected, you cant transmit, says Dr. Meyer. Vaccines prevent infection; therefore, vaccines also prevent onward transmission.

Does Infection Always Mean Transmission

Transmission happens when enough viral particles from an infected person get into the body of an uninfected person. In theory, anyone infected with the coronavirus could potentially transmit it. But a vaccine will reduce the chance of this happening.

In general, if vaccination doesnt completely prevent infection, it will significantly reduce the amount of virus coming out of your nose and mouth a process called shedding and shorten the time that you shed the virus. This is a big deal. A person who sheds less virus is less likely to transmit it to someone else.

This seems to be the case with coronavirus vaccines. In a recent preprint study which has yet to be peer reviewed, Israeli researchers tested 2,897 vaccinated people for signs of coronavirus infection. Most had no detectable virus, but people who were infected had one-quarter the amount of virus in their bodies as unvaccinated people tested at similar times post-infection.

Less coronavirus virus means less chance of spreading it, and if the amount of virus in your body is low enough, the probability of transmitting it may reach almost zero. However, researchers dont yet know where that cutoff is for the coronavirus, and since the vaccines dont provide 100% protection from infection, the Centers for Disease Control and Prevention recommends that people continue to wear masks and social distance even after theyve been vaccinated.

When the U.S. Centers for Disease Control and Prevention changed its guidelines about mask-wearing on May 13, 2021, plenty of Americans were left a little confused. Now anyone who is fully vaccinated can participate in indoor and outdoor activities, large or small, without wearing a mask or physical distancing.

Anthony Fauci, chief medical adviser to President Biden, said the new guideline is based on the evolution of the science and serves as an incentive for the almost two-thirds of Americans who are not yet fully vaccinated to go ahead and get the shot.

But some people cannot be vaccinated because of underlying conditions. Others with weakened immune systems, from cancer or medical treatments, may not be fully protected.
