Not all masks are equal Here’s which ones are best against COVID-19

We recommend that healthcare organizations follow US Centers for Disease Control and Prevention guidance by moving first through conventional, then contingency, and finally crisis scenarios to optimize the supply of respirators. We recommend using the CDC’s burn rate calculator to help identify areas to reduce N95 consumption and working down the CDC checklist for a strategic approach to extend N95 supply.

Filter Efficiency And Fit Are Key For Masks Respirators

The best evidence of mask and respirator performance starts with testing filter efficiency and then evaluating fit . Filter efficiency must be measured first. If the filter is inefficient, then fit will be a measure of filter efficiency only and not what is being leaked around the facepiece.

Filter efficiency

Masks and respirators work by collecting particles through several physical mechanisms, including diffusion and interception and impaction .1 N95 filtering facepiece respirators are constructed from electret filter material, with electrostatic attraction for additional collection of all particle sizes.2

Every filter has a particle size range that it collects inefficiently. Above and below this range, particles will be collected with greater efficiency. For fibrous non-electret filters, this size is about 0.3 micrometers for electret filters, it ranges from 0.06 to 0.1 µm. When testing, we care most about the point of inefficiency. As flow increases, particles in this range will be collected less efficiently.

The best filter tests use worst-case conditions: high flow rates with particle sizes in the least efficiency range. This guarantees that filter efficiency will be high at typical, lower flow rates for all particle sizes. Respirator filter certification tests use 84 L/min, well above the typical 10 to 30 L/min breathing rates. The N95 designation means the filter exhibits at least 95% efficiency in the least efficient particle size range.

Researchers found that surgical masks impede the spread of COVID-19 and that just a few, low-cost interventions increase mask-wearing compliance.

Providing free masks to people in rural Bangladesh was one measure researchers tested to limit the spread of COVID-19. Innovations for Poverty Action

A large, randomized trial led by researchers at Stanford Medicine and Yale University has found that wearing a surgical face mask over the mouth and nose is an effective way to reduce the occurrence of COVID-19 in community settings.

It also showed that relatively low-cost, targeted interventions to promote mask-wearing can significantly increase the use of face coverings in rural, low-income countries. Based on the results, the interventional model is being scaled up to reach tens of millions of people in Southeast Asia and Latin America over the next few months.

The findings were released Sept. 1 on the Innovations for Poverty Action website, prior to their publication in a scientific journal, because the information is considered of pressing importance for public health as the pandemic worsens in many parts of the world.

The researchers also partnered with Innovations for Poverty Action, a global research and policy nonprofit organization.

Studies Of Performance In Real

Before recommending them, it’s important to understand how masks and respirators perform in households, healthcare, and other settings.

Cloth masks as source control

A historical overview of cloth masks notes their use in US healthcare settings starting in the late 1800s, first as source control on patients and nurses and later as PPE by nurses.20

Kellogg,21 seeking a reason for the failure of cloth masks required for the public in stopping the 1918 influenza pandemic, found that the number of cloth layers needed to achieve acceptable efficiency made them difficult to breathe through and caused leakage around the mask. We found no well-designed studies of cloth masks as source control in household or healthcare settings.

In sum, given the paucity of information about their performance as source control in real-world settings, along with the extremely low efficiency of cloth masks as filters and their poor fit, there is no evidence to support their use by the public or healthcare workers to control the emission of particles from the wearer.

We were able to identify only two household studies in which surgical masks were worn by the index patient only, as source control.24,25 Neither of these found a significant impact on secondary disease transmission, although both studies had important limitations.

How Effective Are Masks Really

While face masks arent airtight and therefore cannot keep outeveryrespiratory droplet that may contain disease and bacteria, they are effective at containing a high number of them.

Various studies report different degrees of protection depending on how they were conducted and how data was collected, but a recent meta-analysis of multipleglobal studiesfound that masks are linked to a commanding 53% decrease in COVID-19 transmission when worn properly. Fit Is Important For All Mask Types

How well a mask works also depends on fit. When choosing a mask, keep in mind:

  • the fit of the mask can vary depending on the size and features of your face
  • masks with a flexible nosepiece may provide a better fit over the nose
  • masks with ties or bands that go around the back of the head may provide a better fit
  • if choosing an ear loop-style mask, use one with adjustable ear loops or use the tips below to improve fit
  • respirators are designed to fit snugly on the face, which may allow for a better fit than a medical mask or non-medical mask
  • a respirator worn in the community doesnt need formal fit testing
  • A well-fitting mask should:

    • be large enough to completely and comfortably cover the nose, mouth and chin without gaps and not allow air to escape from edges
    • fit securely to the head with ties, bands or ear loops
    • be comfortable and not require frequent adjustments
    • maintain its shape after washing and drying

    Ways to improve fit include:

    • adjusting ties, bands or ear loops
    • adjusting the flexible nosepiece
    • tying knots in the ear loops and tucking the sides of the mask so that it lies flat
    • using a mask fitter or brace
    • a mask fitter or brace is a device made of flexible material worn over a mask to help provide a snug fit
    • fitters and braces may be made of plastic or silicone
  • layering a well-fitting non-medical cloth mask over the top of a disposable mask to improve the fit

    International Travel Mask Requirements

    Passengers and crew on all flights must wear a face mask during their flight and in Australian airports regardless of whether theyre coming from a safe travel zone or not. They are also encouraged to wear a mask at overseas airports.

    While one could certainly fault Fauci and other officials for not leveling with the American people, its worth noting that their actions at the time still demonstrated how important they considered masks to be. A physician last year told me, nothing convinced me more that I needed to get my hands on some masks for my family then when I saw how panicked Dr. Fauci was when he thought there werent enough masks to go around for doctors and nurses early in the pandemic.

    How Can You Avoid Counterfeits

    N95s, KN95s and KF94s are slightly different in terms of shape and style, but all use high-quality material that filters 94-95 percent of virus. Each type is regulated and certified in a different country. N95s are regulated by the United States National Institute for Occupational Safety & Health . The KN95 is designed to meet Chinese standards, and the KF94 adheres to Korean regulations. Unfortunately, no N95s are made specifically for children, but KN95s and KF94s do exist for kids. They still require the same level or approval as other masks.

    Throughout the pandemic some manufacturers have added misleading words and logos to their labels to make the masks seem like theyve been inspected and approved, even if they arent certified. NIOSH provides some examples of counterfeit masks here, and a list of approved N95s here. The FDA also has two lists of KN95 masks that were authorized for healthcare workers during the shortage of N95s.

    You can also look at this infographic from the CDC that describes the information you should be able to find right on the front of any legitimate N95 mask. Similarly, you should see the name of the manufacturer, model and GB number on the front of a KN95 mask. The KF94s dont have text on the front but should be made in Korea.

    Another option is ordering vetted masks through the nonprofit website, Project N95, instead of suppliers like Amazon that may harbor counterfeits alongside certified masks.

    When removing a used mask, be sure to only touch the ear loops or ties, said Dr. Edje. This is so that you do not get contaminated with virus particles from the outside of the respirator.

    And make sure you do not keep the respirator tucked under your chin for any reason, she said, noting that it must be worn to cover your nose and mouth.

    What Type Of Face Coverings Are The Most Effective

    Filtering masks, such as N95 respirators, are the most effective at filtering out particles in the air while also restricting the number of droplets spread by an individual.

    KN95 respirators are similar to N95 respirators, except they are developed to meet Chinese standards, which differ slightly from U.S. standards. Surgical masks follow next in terms of effectiveness.

    Face coverings made from other materials vary in efficacy, depending on characteristics such as the type of material used, the number of layers present, and fit. Face coverings should completely cover the wearer’s nose and mouth. Those with one-way valves do not provide community protection, since the valve opens to allow easy escape of exhaled breath and any particles that it contains.

    I recently came across this study on cloth masks being ineffective in high-risk situations. The study concluded that moisture retention, reuse, and poor filtration could increase wearers risk of infection. We are being asked to use cloth masks on campus and in other places should we be doing something different?

    The study you cite involved healthcare workers who experienced repeated exposures to the virus due to very close, often physical, contact with infected patients the very definition of high-risk situations. Not surprisingly, the study showed that cloth masks did not protect against infection as well as medical-grade masks.

    But unless you are a frontline healthcare worker, this study does not have much relevance to the precautions you should be taking on campus or elsewhere. Remember, since the virus is spread through respiratory droplets, and since people are most contagious in the 48 hours before symptom onset, our cloth face coverings are meant to protect other people, not ourselves. And there is steadily accumulating evidence that cloth masks perform that function well, by containing respiratory droplets before they can be expelled into the air.
