Face masks and other personal protective equipment are important tools to protect the wearer and others against COVID-19. EPA scientists and others are applying their expertise in aerosol and decontamination research to study the effectiveness of various types of masks for the public and healthcare workers, as well as methods for disinfecting PPE for the public, healthcare workers, and other essential workers, including emergency responders.

Researchers are rigorously testing masks and other PPE in EPAs laboratories in Chapel Hill and Research Triangle Park, North Carolina. They want to know how effectively different materials can filter out virus particles from the air, whether popular do-it-yourself mask modifications work, and which disinfection methods work well for masks and other types of PPE, like face shields and protective suits.

This information will help health care professionals and the public better identify the most effective products and allow hospital personnel to prioritize N95 respirators and identify alternatives during shortages. Two peer-reviewed articles in the Journal of American Medical Association along with one EPA Report, were published in 2020.

Do Face Masks Work Here Are 49 Scientific Studies That Explain Why They Do

AUSTIN Since the beginning of the historic COVID-19 pandemic, one of the most hotly debated subjects has been about masks and whether they actually help slow the spread of the disease. Now, as omicron cuts its way across the globe, the question is back on peoples minds.

At the onset of the pandemic, there was slim evidence to either prove or disprove the effectiveness of mask-wearing to slow the spread of this particular coronavirus because, obviously, COVID-19 was a new disease. Additionally, there was little research focusing on respiratory disease transmission during a global pandemic of COVID-19s magnitude a once-in-a-lifetime worldwide disaster. Naturally, there was little data to go on about the efficacy of mask-wearing during these exact circumstances.

This lack of information was perhaps confusing for many but millions in the scientific community quickly jumped to help the world understand this disease better. They built upon years of existing data on coronaviruses and communicable diseases. While researchers of all 49 studies listed below acknowledge theres still much more data to be explored, they have all acknowledged the efficacy of mask-wearing to some degree at slowing the spread of COVID-19.

Data Lacking To Recommend Broad Mask Use

We do not recommend requiring the general public who do not have symptoms of COVID-19-like illness to routinely wear cloth or surgical masks because:

  • There is no scientific evidence they are effective in reducing the risk of SARS-CoV-2 transmission
  • Their use may result in those wearing the masks to relax other distancing efforts because they have a sense of protection
  • We need to preserve the supply of surgical masks for at-risk healthcare workers.

Sweeping mask recommendationsas many have proposedwill not reduce SARS-CoV-2 transmission, as evidenced by the widespread practice of wearing such masks in Hubei province, China, before and during its mass COVID-19 transmission experience earlier this year. Our review of relevant studies indicates that cloth masks will be ineffective at preventing SARS-CoV-2 transmission, whether worn as source control or as PPE.

Surgical masks likely have some utility as source control from a symptomatic patient in a healthcare setting to stop the spread of large cough particles and limit the lateral dispersion of cough particles. They may also have very limited utility as source control or PPE in households.

Respirators, though, are the only option that can ensure protection for frontline workers dealing with COVID-19 cases, once all of the strategies for optimizing respirator supply have been implemented.

We realize that the public yearns to help protect medical professionals by contributing homemade masks, but there are better ways to help.

What Type Of Mask Is Best

The quality of the mask makes a difference.

The best face mask is a single-use surgical mask. If you are unable to get a surgical mask, any paper or cloth masks are fine to use. The most effective cloth masks are made up of at least 3 layers.

Overall comfort and consistency are the two most important factors in the choice of a mask. Its not going to do any good if you dont want to wear it. In comparing mask materials, as a general rule, surgical masks are typically more effective than cloth masks, however youre encouraged to wear a mask that you find comfortable for wearing throughout the day. Remember to change your mask daily or more frequently if dirty or wet. Cloth masks must be washed each day.

Multilayer Tightly Woven Cloth Masks

The use of multilayer cloth masks can block 5070% of fine droplets and particles. They can also limit the forward spread of droplets and particles that are not captured, notes the CDC. In fact, upwards of 80% blockage has been achieved with cloth masks in some studies, which is about on par with surgical masks as barriers for source control.

Other materials, such as silk masks, may help repel moist droplets. They may also reduce fabric wetting, which can help maintain breathability and comfort for the wearer.

There was a situation on our campus where people were blowing out candles through the mask that was given to the students and were saying, Well, see this is a bad mask, said Dr. Malani. However, the droplets of viruses are different. Just because you can blow through the mask doesnt mean its not useful.

I recently came across this study on cloth masks being ineffective in high-risk situations. The study concluded that moisture retention, reuse, and poor filtration could increase wearers risk of infection. We are being asked to use cloth masks on campus and in other places should we be doing something different?

The study you cite involved healthcare workers who experienced repeated exposures to the virus due to very close, often physical, contact with infected patients the very definition of high-risk situations. Not surprisingly, the study showed that cloth masks did not protect against infection as well as medical-grade masks.

But unless you are a frontline healthcare worker, this study does not have much relevance to the precautions you should be taking on campus or elsewhere. Remember, since the virus is spread through respiratory droplets, and since people are most contagious in the 48 hours before symptom onset, our cloth face coverings are meant to protect other people, not ourselves. And there is steadily accumulating evidence that cloth masks perform that function well, by containing respiratory droplets before they can be expelled into the air.

The study found that the strategy was cost-effective, too. Giving masks to a large population and getting people to use them costs about $10,000 per life saved from COVID, on par with the cost of deploying mosquito nets to save people from malaria, Styczynski said.

“I think that this what we’ve been able to show is that this is a really important tool to be used globally, especially as countries have delays in getting access to vaccines and rolling them out,” she said.

But if you need to be in the same room as someone else, everyone in the room aged 9 and older should wear a mask. For adults and older children, this should be a medical or respirator mask.

For some children, a medical or respirator mask may not fit well on their smaller face. Children who are 9 or older can wear a cloth mask that fits well in these situations. Younger children may not be able to wear any kind of mask.

How To Choose A Cloth Face Mask

When choosing a face mask, make sure it:

  • fits snugly at the side of your face and doesn’t leave any gaps
  • completely covers your nose and mouth
  • has 2 or more layers of washable, breathable fabric – if it is a cloth mask

Avoid masks that:

  • are made of material that makes breathing difficult
  • have valves or vents – these are used to protect you from dust and do not offer protection from COVID-19

Do not wear a visor or face shield instead of a mask. They should only be worn if you have an illness or impairment that makes wearing a face mask difficult.

You do not have to wear a face mask if you have an illness or impairment that would make wearing a face mask difficult.

Face masks are not recommended for anyone who:

Educational And Behavioral Interventions

The researchers wanted to explore whether it was possible to increase mask-wearing in Bangladeshi villages through a variety of educational and behavioral interventions over an eight-week study period: Free cloth or washable, reusable surgical masks were given to people at home and in marketplaces, mosques and other public spaces notable Bangladeshi figures, including the prime minister, a star cricket player and a leading imam, provided information about why wearing a mask is important people appearing in public places without masks were reminded to wear masks and community leaders modeled mask-wearing.

The villages were selected by researchers at Innovations for Poverty Action Bangladesh. The researchers paired 600 villages countrywide based on population size and density, geographic location, and any available COVID-19 case data. For each of the 300 pairs of villages, one was randomly assigned to receive the interventions while the other served as a control and received no interventions. Two-thirds of the intervention villages received surgical masks, while the other one-third received cloth masks. In total, 178,288 people were in the intervention group, and 163,838 people were in the control group.

We saw the largest impact on older people who are at greater risk of death from COVID-19.
