BEIJING, June 29 – Many countries from China to Indonesia and Brazil rely heavily on Chinese vaccines to inoculate their people against COVID-19, but there are growing concerns about whether they provide enough protection against the Delta variant, first identified in India.

Below are views from China’s health experts about the effectiveness of home-grown vaccines against the Delta, which is becoming the globally dominant variant, and virus preventive measures China is taking.

DO CHINESE VACCINES WORK AGAINST DELTA?

China has not provided vaccine effectiveness results against the variant based on large-scale data in clinical trials or real-world use, nor offered detailed information from lab tests, but Chinese experts are urging people to get inoculated as soon as possible.

The lack of detailed data on the Chinese vaccines against the Delta has hobbled any meaningful peer reviews by foreign experts.

Researchers found that Chinese vaccines are somewhat effective in reducing the risk of symptomatic and severe cases caused by Delta, Zhong Nanshan, a epidemiologist who helped shape China’s COVID-19 response, told reporters.

It is based on analysis of infections in Guangzhou city, and Zhong told Reuters the results are preliminary and the sample size is small.

All the vaccines coming out of the U.S. and Europe currently in advanced clinical trialsas well as those receiving Operation Warp Speed fundinguse high-tech methods to generate immunity to a part of the SARS-CoV-2 virus known as the spike protein. Spike proteins, which protrude from the core of the virus and give it its crown-like shape, help the pathogen bind to human cell receptors and infect your body. By varying techniques, these vaccines all aim to prime your body to attack the spike protein before it can attach to your bodys cells in the first place.

Are The Chinese Vaccines Effective

In clinical trials across the world, Sinovac and Sinopharm’s inactivated virus vaccines have been shown to be 50% to 79% effective in preventing symptomatic Covid infection.

But they are still highly effective in preventing Covid hospitalisations or deaths – studies found Sinovac’s jab was 100% effective in Brazil and 96 to 98% effective among Indonesian medical workers.

One is that the Chinese vaccines, like many other vaccines, may wane in efficacy over time. A Thai study released this week found that antibodies in those fully vaccinated with Sinovac decline by half every 40 days.

Another is that the clinical trials had smaller datasets compared to real-world infections, particularly in Indonesia which is seeing soaring daily infection numbers in the tens of thousands.

It could also be due to the more infectious Delta variant, which has been detected in 60% of recent cases in Indonesia and 26% of cases in Thailand’s capital Bangkok.

There is no public data yet on the Chinese vaccines’ efficacy against any of the Covid variants. But preliminary studies have suggested that inactivated virus vaccines, like Sinopharm and Sinovac’s, could offer 20% less protection against the Delta variant than against the original virus, according to Prof Cowling.

There are also no reports yet of breakthrough infections in China, where more than 630 million have taken at least one shot of a Chinese vaccine. It is not known how many of them are fully vaccinated.

What Share Of The Population Has Been Fully Vaccinated Against Covid

The following chart shows the share of the total population that has been fully vaccinated against COVID-19. This represents the share that have received all doses prescribed by the vaccination protocol. If a person receives the first dose of a 2-dose vaccine, this metric stays the same. If they receive the second dose, the metric goes up by 1.

This data is only available for countries which report the breakdown of doses administered by first and second doses.

Discussion Over Travel Benefits For Vaccinated People

Greece and Israel signed an agreement to that will allow vaccinated tourists to travel between their two countries withthout restrictions. In Europe, whether vaccinated people should be the first to be allowed to travel again is controversial. While Germany is still reluctant, some countries already allow easier entry with a vaccination certificate, including Estonia, Poland and Iceland.

Who Should Be Vaccinated First

While COVID-19 vaccine supplies are limited, health workers at high risk of exposure and older people should be prioritized for vaccination.

The vaccine is not recommended for persons younger than 18 years of age, pending the results of further studies in that age group.

Countries can refer to the;WHO Prioritization Roadmap;and the;WHO Values Framework;as guidance for their prioritization of target groups.

On the other side of the world, President Xi Jinping would like nothing more than to be first with a safe and effective vaccine. Hed be able to give it to his own population, pulling his economy and society back on track, while pointing out to the world that the U.S. is still struggling.

And then, he can look abroad.

Top Chinese officials and drugmakers are already wielding the vaccine as a tool of diplomacy, promising first access to countries of strategic interest. Priority will go to the Philippines and Indonesia, countries that have quarreled with China over territory in the South China Sea, and to Brazil, Chinas most important political and economic partner in Latin America one that has been gravitating to Trumps orbit since President Jair Bolsonaro took office last year. One Chinese pharmaceutical company, CanSino, is talking to several countries, including Pakistan and some in Latin America, about distributing the vaccine to essential workers on an emergency basis even before Phase III trials are complete, but no agreements have been reached.

But a Covid-19 vaccine gives Beijing a chance to test out new waters: What the Chinese look for in many places is just an entrance. They push on open doors, Price says. And having a vaccine that no one else has is certainly a way to open doors around the world to countries who have been reluctant to allow the Chinese the same sort of access theyve garnered elsewhere.

How Efficacious Is The Vaccine

A large multi-country Phase 3 trial has shown that 2 doses, administered at an interval of 21 days, have an efficacy of 79% against symptomatic SARS-CoV-2 infection 14 or more days after the second dose. Vaccine efficacy against hospitalization was 79%.

The trial was not designed and powered to demonstrate efficacy against severe disease in persons with comorbidities, in pregnancy, or in persons aged 60 years and above. Women were underrepresented in the trial. The median duration of follow-up available at the time of evidence review was 112 days.

Two other efficacy trials are under way but data are not yet available.

One year after the Covid-19 virus was first identified in Wuhan, China has gone from being the worlds epicenter of the pandemic to one of the countries with the lowest reported infection rates. But alongside this transformation is the Chinese governments lack of transparency, and its reliance on censorship and propaganda.

The pandemic started with Beijing withholding information from the public, underreporting cases of infection, downplaying the severity of the infection, and dismissing the likelihood of person-to-person spread. Now, the authorities have moved to detain vaccine safety advocates, censor information critical of the Chinese-made vaccines, and spread disinformation about non-Chinese vaccines.

Beijings Covid-19 vaccine funding and development process has been shrouded in opacity. While other governments, notably the US and the European Commission, have begun publishing redacted copies of the contracts they have signed with vaccine producers, Chinese authorities have yet to take this step.

As early as July 2020, Chinese authorities began to vaccinate people in high risk professions, including medical professionals and border workers. Some state-owned companies required their overseas-based Chinese workers to be vaccinated before they returned to their jobs abroad. Some workers expressed concerns over the lack of transparency and dearth of safety and efficacy data before the vaccines official approval.

Which Of Chinas Vaccines Have Been Approved Outside Of China

Sinopharms first vaccine has received the most emergency use approvals so far, nearly 30, including in Bahrain, Guyana, Hungary, Serbia, and the UAE. Hungary was the first EU country to approve use of a Chinese vaccine, with Prime Minister Viktor Orbán among those who have received it.

Several countries have approved Sinovacs CoronaVac jab for emergency use, including Brazil, Chile , Indonesia, Laos, Mexico, and Turkey.

Mexico and Pakistan have given emergency use approval to the CanSinoBIO vaccine.

The Anhui Zhifei Longcom vaccine has received approval for use in Uzbekistan.

China Faces Difficulties In Expanding Covid

BEIJING, Sept 3 – China is facing growing difficulties in expanding its mass COVID-19 vaccination drive, but it will continue to inoculate more people and step up the programme of booster shots, a health official said on Friday.

Zheng Zhongwei, an official at the National Health Commission, did not specify the obstacles but stressed that those who have not been vaccinated could not rely on being protected by those who have had the shots amid concerns over the highly transmissible Delta variant.

“Recently, as came to the later stage, it has become increasingly difficult,” Zheng said at a health forum.

He said China has given full doses to about 900 million people, or more than 60% of its 1.4 billion population, but cited some experts saying China may need a vaccination rate higher than 80%.

China administered around 13 million doses per day on average in July and August, slower than June’s daily average of 19 million.

Currently people are recommended to take booster doses of the same type of technology as their first vaccinations, but more options might be available once studies on using different shots as boosters yield results, Zheng said.

China has supplied 1 billion doses of COVID-19 vaccines to the rest of the world, Zheng said.

The Limits Of Zero Tolerance

This calls into question whether Chinas policy of zero tolerance to COVID-19 can work against the highly infectious Delta variant.

Countries, such as Singapore and the United Kingdom, have adopted a more pragmatic attitude that many have referred to as learning to live with the virus.

This approach argues that societies must accept a steady rate of ongoing infections most of them relatively mild as a result of vaccination in order to sustain their economies and protect the well-being of the majority of citizens.

According to Vincent Ni, China affairs correspondent of the U.K.-based newspaper The Guardian, the Chinese virologist Dr. Zhang Wenhong, Ph.D. popularly known as Chinas Dr. Fauci has said the outbreak in Nanjing should serve as food for thought for the future of our pandemic response.

Dr. Wenhong, who is head of the center of infectious diseases at Huashan Hospital of Fudan University, wrote in an essay:

The data tell us that even if each of us were to be vaccinated in the future, COVID-19 would still be endemic, but at a lower level with a lower fatality rate. After the liberalization of vaccines, there will still be infections in the future.

Which Countries Are Administering China’s Vaccines

More than 80 countries are using China’s Covid vaccines, including many in Asia, among them Indonesia, Thailand, Malaysia, Vietnam and the Philippines.

However, some countries that opted for the Chinese jabs have high vaccination rates, and are still seeing a surge in infection numbers.

For instance, Chile re-imposed a curfew and brought back restrictions on travelling in response to the Delta variant, which is more transmissible than previous variants.

More than 70% of Chileans are fully vaccinated, most with the Sinovac vaccine.

The Seychelles and Mongolia, meanwhile, have recently recorded some of the highest increase in cases per capita, despite their small populations.

Both are relying heavily on Sinopharm and their vaccination programmes are advanced: 68% of adults are fully vaccinated in the Seychelles and 55% in Mongolia.

Thailand has changed its vaccine policy to mix China’s Sinovac with the AstraZeneca vaccine in a bid to boost protection after hundreds of medical workers caught Covid despite being fully vaccinated with Sinovac.

Meanwhile in Indonesia, the main doctors and nurses’ association said at least 30 healthcare workers died despite receiving two doses of the Sinovac vaccine.

The country is now planning to switch to different Covid-19 vaccines for second doses or administer booster shots to increase efficacy.
