In the absence of a reliable death registration system, the government’s Integrated Disease Surveillance Program has been collecting data on COVID cases and deaths from testing laboratories and hospitals.

However, the IDSP’s major drawback is that it has no way of tracking deaths outside hospitals.

“In the current scenario, India’s soaring official COVID-19 count may just represent the tip of an iceberg. Because of low testing rates outside big cities, the actual caseload, and deaths, could be anything from 10 to 30 times higher,” Vikas Bajpai of the Progressive Medicos and Scientists Forum told DW.

Positivity, too, has been high with nearly one in five in the country and one in three in Delhi who have been tested turning out COVID positive.

“Far better reporting of COVID-19 deaths is needed. Daily or weekly reporting of the total death counts by age and sex as well as by each municipality would help track if there is a spike in presumed COVID-19 deaths,” Prabhat Jha of the Center for Global Health Research in Toronto told DW.

Why The Pandemic Could Change The Way We Record Deaths

But COVID death undercounts are happening almost everywhere. In the United States, the official toll is 500,000 but the real number is closer to 700,000, says Ali Mokdad of the Institute for Health Metrics and Evaluation . The group’s website has a global rundown that estimates “excess mortality” in many countries during the pandemic.

When counting “excess deaths,” the cause of death is not part of the data set. But during a health crisis like the pandemic, the assumption is that these additional deaths are part of the COVID-19 toll, says Mokdad. They reflect not only those who died of the virus but those who might have died, say, of heart disease or diabetes because they were afraid to seek treatment during lockdowns, and those who killed themselves due to pandemic stresses, he adds.

Surging Cases Lead To Delayed Results

Shahid Jameel, a virologist and director of the Trivedi School of Biosciences at Ashoka University, also maintains that the real death toll is higher than the recorded one, based on reports from cremations and burial sites.

He pointed out that the surging demand for COVID tests has created backlogs, causing labs that issued results within hours to now take days to deliver them.

“One problem that is happening is that test results are taking too long to come. It has happened to my cousin in Uttar Pradesh. He was tested on April 13 and his test result has still not come,” Jameel told DW.

“And his will not be reported as a COVID death even though he had all symptoms, including a very high lung infection score and high inflammation markers in blood. There are thousands of cases like that.”

More than 15,000 COVID

India’s COVID-19 tally crossed the 22-million mark on Sunday, reaching 22,296,414 infections, as 403,738 new cases were registered in the past 24 hours, the Health Ministry said.

As many as 4,092 deaths since Saturday morning were recorded, taking the total to 242,362, added the ministry. It marked the fourth consecutive day over 4,000 deaths were recorded in 24 hours.

Covid Death Toll In India Likely Far Higher Than Official Record Research Says

Several million Covid-19 deaths have most likely gone unreported in India, according to a series of recent studies that suggest the country’s death toll from the virus is far higher than what has been officially tallied by the government.

A team of researchers in Canada, India and the United States estimated that roughly 3 million Covid deaths during the country’s first and second waves of infection remain unaccounted for by Indian officials. The findings, releasedThursday in the journal Science,appear to confirm long-held suspicions among epidemiologists that India’s official record of 483,178 Covid deaths may have significantly undercounted the virus’ true devastation.

“Earlier in the pandemic, we had this ‘Indian paradox,’ where there was widespread infection but not many deaths,” said Dr. Prabhat Jha, a professor of epidemiology at the University of Toronto in Canada, who led the research. “It was a bit of a puzzle as to why that was happening, but when we started looking into it, there were obviously missing deaths.”

India has confirmed more than 35 million cases of Covid-19 since the start of the pandemic, according to the country’s Ministry of Health and Family Welfare.

Jha and his colleagues found that as many as 3.4 million Covid deaths were probably undercounted from June 2020 to July 2021. Many of those deaths occurred last spring, Jha said, when India was hit especially hard by the delta variant.

JUST A FEW months after Indias public-health systems collapsed under a tsunami of covid-19 infections, the country is starting to feel it has something to celebrate. In good time for the season of Hindu holidays now under way, on October 21st Narendra Modi, the prime minister, declared that India had administered its billionth dose of vaccine. In cities such as Delhi and Mumbai the dedicated covid-19 wards are virtually empty . A doctor at a major government hospital quipped that it must now be harder to contract the virus in Delhi than anywhere abroad. Indias economy is still wonky, but for most of the summer stockmarket indices have been reaching new heights. Yet this palpable sense of relief is hard to square with the recent memory of mass death, when fields of bodies were buried hastily along the Ganges. How did India manage to beat covid-19?

Alternative Sources On Covid

The aim of this research is to number Covid-19 deaths since the beginning of the pandemic by identifying new sources on Covid-19 mortality from which to develop reliable estimates of deaths in India. The objective is not to compute Covid-19 excess mortality, which requires a complex set of hypotheses on expected mortality during a pandemic and representativeness of existing figures. In this paper, we have tried to evaluate the number of deaths directly attributable to Covid-19, but we could not assess the overall impact of the pandemic on Indian mortality.

Several sources provide the number of Covid-19 deaths for specific communities, organizations, or localities. For this purpose, we have examined most of the available samples of mortality statistics related to COVID-19 and verified their reliability, completeness, and representativeness. We have also focused on the entire pre-vaccination period as samples might have not been affected uniformly by the vaccination drive. The selection criteria were the following:

  • Samples based on well-circumscribed populations .
  • Reliable estimation of the Covid-19 deaths
  • Samples large enough for mortality estimates .
  • Populations with information on their age and sex structures
  • Samples covering the entire pre-vaccination period from March 2020 to May 2021.
  • Samples representative at all-India level .

    The Pandemics True Death Toll: Millions More Than Official Counts

    Last years Day of the Dead marked a grim milestone. On 1 November, the global death toll from the COVID-19 pandemic passed 5 million, official data suggested. It has now reached 5.5 million. But that figure is a significant underestimate. Records of excess mortality a metric that involves comparing all deaths recorded with those expected to occur show many more people than this have died in the pandemic.

    Working out how many more is a complex research challenge. It is not as simple as just counting up each countrys excess mortality figures. Some official data in this regard are flawed, scientists have found. And more than 100 countries do not collect reliable statistics on expected or actual deaths at all, or do not release them in a timely manner.

    Demographers, data scientists and public-health experts are striving to narrow the uncertainties for a global estimate of pandemic deaths. These efforts, from both academics and journalists, use methods ranging from satellite images of cemeteries to door-to-door surveys and machine-learning computer models that try to extrapolate global estimates from available data.

    The uncertainty in this estimate is a discrepancy the size the population of Sweden. The only fair thing to present at this point is a very wide range, says Sondre Ulvund Solstad, a data scientist who leads The Economists modelling work. But as more data come in, we are able to narrow it.

    At the time the results of each survey were released, they indicated infection prevalence between 13.5 and 28.5 times higher than Indias reported case counts at those points in the pandemic. The severity of underreporting may have increased or decreased since the last serosurvey was completed, but if it has held steady, that would suggest that almost half of Indias population may have had the virus.

    Dr. Shioda said that even the large multipliers found in the serosurveys may rely on undercounts of the true number of infections. The reason, she said, is that the concentration of antibodies drops in the months after an infection, making them harder to detect. The number would probably be higher if the surveys were able to detect everyone who has, in fact, been infected, she said.

    Those people who were infected a while ago may have not been captured by this number, Dr. Shioda said. So this is probably an underestimate of the true proportion of the population that has been infected.

    Like nearly all researchers contacted for this article, however, Dr. Shioda said the estimator provided a good way to get a sense of the wide range of possible death tolls in India.

    The Study States That During This Period Covid Likely Doubled The All

    Indiaâs coronavirus deaths top 200,000

    Indias actual Covid-19 toll could be six times the reported figure, with a study published in the Science journal on Thursday estimating it to be 3.2 million. A total of 483,178 Covid-19 deaths have been officially recorded so far since the pandemic began.

    Nearly 71% or 2.7 million of the estimated deaths occurred between April and June last year, when the Delta wave was ravaging through the country, the researchers found. In fact, the study states that during this period, Covid likely doubled the all-cause mortality .

    Indias official cumulative Covid death count of 0.48 million implies a Covid death rate of approximately 345/ million population, about one-seventh of the US death rate. Indias reported Covid death totals are widely believed to be under-reports because of incomplete certification of Covid deaths and misattribution to chronic diseases and because most deaths occur in rural areas, often without medical attention, the study states.

    The study was conducted by researchers from India, Canada, and the US, including Dr Prabhat Jha from the Centre for Global Health Research at the University of Toronto and Dr Paul Novosad from the department of economics at Dartmouth College.

    New Surveys Corroborate Earlier Estimates That The Number Is Some Six Times Higher

    AS INDIAS FEARSOME second wave of covid-19 recedes, the fact that fewer are falling ill is not the only cause for relief. On June 7th Narendra Modi, the prime minister, announced a policy switch that should make it easier for more Indians to get vaccinated. Instead of forcing individual states to compete in procuring vaccines, the central government will now itself buy all the jabs and give out 75% free of charge. The move may restore some faith in Mr Modis leadership, after his early promises of a world-beating vaccination programme crashed into the grim reality of surging deaths and shrinking supplies of shots.

    Yet as Indians emerge from lockdowns, it is not easy to shake off the gloom. True, the official death toll has fallen steadily for the past month, to half its peak of over 4,000 a day in mid-May. But evidence continues to accumulate that the governments numbers represent a disturbingly small fraction of the real figure. This discrepancy does not just mean that the true level of Indias suffering has been glossed over. It has made the crisis worse, for instance by causing authorities to underestimate demand for oxygen and drugs.

    Indias Gujarat Admits To More Covid Deaths Than Official Tally

    The acknowledgement lends weight to fears that the countrys actual toll is much higher than reported.

    Indias western state of Gujarat has acknowledged there were more COVID-19 deaths than its official tally, according to a court document, lending weight to fears that the countrys actual toll is much higher than reported.

    Many media reports, based on figures collected from crematoriums and cemeteries, said that during Indias record second wave of cases between April and June, states including Gujarat undercounted deaths as many people died at home due to a severe shortage of hospital beds and oxygen.

    Gujarat, Prime Minister Narendra Modis home state, told the Supreme Court it had received 22,557 applications as of Thursday from families of the dead seeking compensation and 16,175 had been approved, according to an affidavit filed on Monday.

    Its reported death count is 10,099, according to the states latest health bulletin.

    A Gujarat official with direct knowledge of the matter said applications had now swelled to more than 40,000 and nearly half had been approved. All of them would get 50,000 rupees each.

    India has reported a total of 475,636 COVID-19 deaths, including many revised figures from states that have come under pressure from courts to accurately represent the scale of the disaster.

    The main opposition Congress party said that it believed the actual death toll to be still higher.
