    Bring your CDC COVID-19 Vaccination Record card with you to your appointment if you need another shot of COVID-19 vaccine so your provider can fill in the information about your additional shot.

    If you have lost your CDC COVID-19 Vaccination Record card or dont have a copy, contact your vaccination provider directly to access your vaccination record.

    • If you cannot contact your vaccination provider site directly, contact your state health departments immunization information system . Vaccination providers are required to report COVID-19 vaccinations to their IIS and related systems.
    • If you enrolled in v-safe or VaxText after your first vaccine dose, you can access your vaccination information using those tools.
    • If you have made every effort to locate your vaccination information, are unable to get a copy or replacement of your vaccination card, and need another COVID-19 vaccine dose, talk to a vaccination provider.

    You should get a small white card at your vaccination appointment with your name, birth date, name of the vaccine you received and the date it was administered. If you receive the Pfizer or Moderna vaccine, bring your card when you get your second shot. You may need your vaccine card to schedule a third vaccine dose, for certain immunocompromised people, or a booster shot.     When Can We Stop Masking And Distancing

    Masks continue to be required in some settings, including health care, long-term care and correctional facilities. Per federal requirements, masks also continue to be required on public transportation and in transportation hubs. Local health jurisdictions, some school districts, and individual businesses may still choose to require masks. If you are in a setting where COVID-19 safety measures are in place, please follow them. The goal of these safety measures is to protect staff, employees, students, yourself, and others.     Are You Eligible For A Vaccine Or Booster

    In May, the CDC authorized the Pfizer vaccine for those ages 12 to 15. In October, the CDC did the same for those ages 5 to 11. Adults ages 18 and up can receive the Pfizer, Moderna or Johnson & Johnson vaccine.

The pediatric dose for ages 5-11 is a third of the normal Pfizer vaccine, and a Moderna booster dose is a half-dose of the Moderna vaccine.

    A third dose for people who are immunocompromised and may not have had a robust immune response is the same amount as a full dose.
