La County Will Stop Using Curative Coronavirus Test After Concerns From The Fda

Los Angeles County health officials said Sunday they will stop providing a commonly used coronavirus test after federal regulators raised questions about its accuracy.

The decision affects only a small number of county-supported mobile testing sites. County health officials had already discontinued the broad use of oral swab tests produced by Silicon Valley start-up Curative over the summer because of concerns about too many false negatives.

The use of Curative oral swab tests at the city of Los Angeles’ drive-through testing sites are unaffected by Sunday’s decision, although Mayor Eric Garcetti’s office said separately that the Dodger Stadium site would be turned into a vaccine distribution center this week, ending testing operations there Monday. Garcetti has defended the tests as broadly effective and said that moving away from them could lead to fewer people being diagnosed and greater spread of the virus.

Guidance issued last week from the Food and Drug Administration warned healthcare providers and patients that the test made by Curative carries a risk of false results, particularly false negative results.

To reduce the risk of false negatives, the Curative test should be used only on symptomatic individuals within 14 days of COVID-19 symptom onset, and the swab should be observed and directed by a healthcare worker, the FDA said.

The True Accuracy Of Tests For Covid

Unfortunately, its not clear exactly how accurate any of these tests are. There are several reasons for this:

  • We dont have precise measures of accuracy for these tests just some commonly quoted figures for false negatives or false positives, such as those reported above. False negative tests provide false reassurance, and could lead to delayed treatment and relaxed restrictions despite being contagious. False positives, which are much less likely, can cause unwarranted anxiety and require people to quarantine unnecessarily.
  • How carefully a specimen is collected and stored may affect accuracy.
  • Because these tests are available by EUA, the usual rigorous testing and vetting has not yet happened, and accuracy results have not been widely published.
  • A large and growing number of laboratories and companies offer these tests, so accuracy may vary.
  • All of these tests are new because the virus is new. Without a long track record, assessments of accuracy can only be approximate.
  • We dont have a definitive “gold standard” test with which to compare them.

At the testing site, you will be placed in a testing line to be tested. Once you are checked in and receive your testing kit, remove the swab from its packaging being careful to not touch the swab on any surface before or after the sample collection. Hold the swab on its scoreline and gently insert it into one of your nostrils about half an inch until the tip of the swab is no longer visible. Rotate the swab around the walls of the nostril at least three times to ensure saturation. Repeat in your other nostril. You will then uncap the lid of the tube in the kiosk, place the swab in the tube, close the tube and turn it upside down several times to mix the specimen with tube solution.     Fda Issues Alert About False Negative Results With Curative Covid

    The Food and Drug Administration on Monday issued an alert to patients and healthcare providers warning them of a risk of false negative results with a SARS-CoV-2 laboratory-based test from San Dimas, California-based Curative.

    The agency asserted that the test, to which it granted Emergency Use Authorization in April, must be performed in accordance with its labeling to ensure correct results.

    “When the test is not performed in accordance with its authorization or as described in the authorized labeling, there is a greater risk that the results of the test may not be accurate,” the FDA wrote in a safety communication.

    The PCR-based test is authorized for use on throat swab, nasopharyngeal swab, nasal swab, or oral fluid specimens for the detection of nucleic acid from SARS-CoV-2. The FDA statement highlighted that use of nasal swabs and oral fluid specimens is limited to symptomatic individuals within 14 days of COVID-19 symptom onset, and specimen collection must be directly observed and directed during the sample collection process by a trained healthcare worker at the specimen collection site.

    “A negative result does not rule out COVID-19 and should not be used as the sole basis for treatment or patient management decisions,” the agency said. “A negative result does not exclude the possibility of COVID-19.”

    For Curative The Pandemic Has Meant Booming Business

    Curative is the latest in a string of enterprises founded by the 25-year-old Turner, who dropped out of a biochemistry program at Oxford University in 2016, moved to California, and joined Y Combinator, the famed Silicon Valley startup incubator.

    The premise of his first venture, TL Biolabs, was to sell genomic data about cows to their farmers. When demand failed to materialize, he launched Shield Bio, a lab-testing business with a focus on infectious diseases. That didnt pan out, either. In January 2020, he cofounded Curative to test for sepsis, a dangerous blood infection.

    By March, it was becoming clear that the coronavirus was a problem and that the US was woefully unprepared to detect it. So in April, Curative secured an emergency use authorization from the FDA to do SARS-CoV-2 testing, an expedited form of approval from the agency that requires less data.

    Its three labs, in San Dimas, California, Washington, DC, and outside Austin, Texas, now process about 1 million tests a week from what the company says are 10,000-plus testing sites. Curative, which is headquartered in San Dimas, now has more than 4,000 employees. The scale is just crazy at this point, Turner told the Los Angeles Business Journal this month.

    As of Friday, Curative had partnerships with at least six states, four cities, one county, a hotel chain, and the Department of Defense, according to the company.

    What does the test look for? These blood tests identify antibodies that the bodys immune system has produced in response to the infection. While a serologic test cannot tell you if you have an infection now, it can accurately identify past infection.

    How long does it take to get results? Results are usually available within a few days.

    What about accuracy? Having an antibody test too early can lead to false negative results. Thats because it takes a week or two after infection for your immune system to produce antibodies. The reported rate of false negatives is 20%. However, the range of false negatives is from 0% to 30%, depending on the study and when in the course of infection the test is performed.

    Research suggests antibody levels may wane over just a few months. And while a positive antibody test proves youve been exposed to the virus, its not yet known whether such results indicate a lack of contagiousness or long-lasting, protective immunity.
