Pulse Oximeters: Practicalities And Cautions

Home pulse oximeters have long been used in primary care settingsmost usually for monitoring chronic lung disease and heart failure.22 Devices cost £15-35 when sold commercially, but substantial savings can be made on bulk purchases. They are relatively simple and quick to use, though not everyone is able to understand or operate them.

Finger pulse oximeters are generally accurate and reliable , however, most smartphone oximeters are unreliable and should not be used.2324 The Apple watch has an inbuilt oximeter that operates on similar principles to a standard finger oximeter but is not designed for medical use .

Pulse oximeter readings should be taken on a warm finger, with the patient in the upright position and resting. The instrument should be left to stabilise for a minute before confirming the reading. Falsely low readings may occur if the patients peripheries are cold, if there is poor perfusion owing to hypotension, hypovolaemic shock, or cardiac failure, if the patient has tattoos, nail polish, or false nails, when the patient is supine , and at low levels of oxygen saturation.2324 Readings may also be falsely low in patients with anaemia, sickle cell disease, and other haemoglobinopathies. This article relates mainly to those over 16 years, however, oximeters are accurate in children, though it is important to use a paediatric oximeter and to use it on a finger .

Prone Positioning For Nonintubated Patients

Although prone positioning has been shown to improve oxygenation and outcomes in patients with moderate-to-severe ARDS who are receiving mechanical ventilation,7,8 there is less evidence regarding the benefit of prone positioning in awake patients who require supplemental oxygen without mechanical ventilation. In a case series of 50 patients with COVID-19 pneumonia who required supplemental oxygen upon presentation to a New York City emergency department, awake prone positioning improved the overall median oxygen saturation of the patients. However, 13 patients still required intubation due to respiratory failure within 24 hours of presentation to the emergency department.9 Other case series of patients with COVID-19 requiring oxygen or NIPPV have similarly reported that awake prone positioning is well-tolerated and improves oxygenation,10-12 with some series also reporting low intubation rates after proning.10,12

A prospective, multicenter observational cohort study in Spain and Andorra evaluated the effect of prone positioning on the rate of intubation in COVID-19 patients with acute respiratory failure receiving HFNC. Of the 199 patients requiring HFNC, 55 were treated with prone positioning. Although the time to intubation was 1 day in patients receiving HFNC and prone positioning versus 2 days in patients receiving only HFNC , the use of awake prone positioning did not reduce the risk of intubation .13

Dr. Connolly: Pulse oximeters can have falsely low readings if a person has circulatory issues with poor blood flow to the extremities, such as very cold hands, intrinsic vascular disease or Raynaud’s phenomenon. In addition, fake nails or certain darkly colored nail polish, such as black or blue, can distort the readings.

I always recommend that people measure at least one finger per hand to confirm the number.

How Accurate Are Pulse Oximeter Readings

Most pulse oximeters are accurate to within 2% to 4% of the actual blood oxygen saturation level. This means that a pulse oximeter reading may be anywhere from 2% to 4% higher or lower than the actual oxygen level in arterial blood.;;

A number of factors can impair the functioning or accuracy of a pulse oximeter. Nail polish and artificial nails may block the red and infrared light emitted by the device.
