Federal approval and regulation of diagnostic tests
Only COVID-19 tests approved by Health Canada can be imported or sold in Canada. Since this is usually a lengthy process, on March 18, the Minister of Health Hajdu issued an interim order to allow expedited access to COVID-19-related medical devices for use by healthcare providers, including diagnostic test kits. The same day, the first commercial tests were approved, RT-PCR tests from Roche and Thermo Fisher. Another 13 diagnostic products have since been approved, all based on Nucleic Acid tests. As of April 30, 21 diagnostic device applications were listed as submitted by Health Canada.
National Microbiology Lab
Canada’s National Microbiology Lab in Winnipeg performs diagnostic testing for and research into COVID-19. Samples from suspected cases early in the pandemic were sent by provinces and territories to this national lab for testing, either as the sole test or as a check of an in-province test result. The lab diagnosed the first confirmed case in Canada on January 27, 2020. Since then, provinces and territories have established their own testing capacity but have occasionally sent samples to the national lab for a second test as a check.

BEIJING, CHINA JANUARY 21: A Chinese girl wears a protective mask as she is held by a relative as they wait to board a train at Beijing Railway station before the annual Spring Festival on January 21, 2020 in Beijing, China. The number of cases of a deadly new coronavirus rose to nearly 300 in mainland China Tuesday as health officials stepped up efforts to contain the spread of the pneumonia-like disease which medicals experts confirmed can be passed from human to human. The number of those who have died from the virus in China climbed to six on Tuesday and cases have been reported in other parts of Asia including in Thailand, Japan, Taiwan and South Korea.
With the help of methods used commonly in conservation science, a group of scientists estimate that the first case of Covid-19 emerged between October and mid-November 2019 in China. A new analysis published in the journal PLOS Pathogens, David Roberts from the University of Kent, U.K., and colleagues suggest the estimated date of origin was November 19.
While Covid-19 continues to wreak havoc in India, Nepal, and several other countries, the quest to find answers about its origins remains elusive. Scientific evidence points towards the origins of the SARS-CoV-2 virus as a zoonotic spillover that occurred before early December 2019 before it spread globally.

Increase Testing Capacity And Strengthen Disease Surveillance
Since May 31 when Ghana eased the lockdown, new daily cases are on the rise. Since June 15, students have also started coming back to school, which could add to the risk of increase in viral transmission. To better control the spread of COVID-19, it is essential for the government to ramp up the capacity and scope of contact tracing, testing, and routine surveillance. Second, restrictions should be eased slowly in a stepwise approach with close monitoring. Any decision to reduce restrictions should be based on surveillance of the COVID-19 incidence and severity. Third, more test labs should be established as the current rate of testing is not commensurate with the growing demand for COVID-19 testing. Fourth, the government should sanitize public spaces and effectively implement public health measures such as social distancing and mask wearing. One possible strategy that could be helpful is for the government to monitor cross-city mobility and uniformly test or quarantine people coming from COVID-19 hotspots.
England Entered A National Lockdown

Mixing Of Cohorts At Lunch And Recess Leading To Covid
Ottawa’s medical officer of health suggests the mixing of cohorts at lunch and recess in schools is leading to new cases of COVID-19 in children.
Ottawa Public Health closed two schools this week due to COVID-19 cases, while there are nine current outbreaks in elementary schools.
On Tuesday, St. Benedict Elementary School was closed due to “evidence of the spread of COVID-19 to several cohorts in the school.” Cases were first identified in kindergarten cohorts. One parent tells CTV News Ottawa a positive test result was reported in her child’s Grade 4 class.
On Thursday, École élémentaire catholique Marius-Barbeau was closed due to evidence of the virus spreading to several different cohorts.
“What we are seeing is that there’s more mixing of students than there was last year. When I say mixing of students, I mean the groups that children and youth are part of are called cohorts and now those cohorts are sometimes mixing at recess or mixing at lunch, and this is leading to greater exposures,” said Dr. Vera Etches on Friday.
“We’re talking with the school boards about how to mitigate that, I think there’s some more things that can be done to reduce the number of close contacts that people have.”
Fifty-eight schools have at least one active case of COVID-19 in Ottawa.

The outbreak spanning the globe began in December, in Wuhan, China.
The novel coronavirus outbreak, which began in Wuhan, China, in December, has expanded to touch every corner of the globe. Millions of people around the world have been sickened and hundreds of thousands of others have died.
Heres a timeline of how the outbreak has unfolded so far:
Health authorities in China confirm that dozens of people in Wuhan, China, are being treated for pneumonia from an unknown source. Many of those sickened had visited a live animal market in Wuhan, but authorities say there is no evidence of the virus spreading from person to person.
Chinese state media reports the first death from novel coronavirus, a 61-year-old man who had visited the live animal market in Wuhan.
A man in his 30s from Washington state, who traveled to Wuhan, is diagnosed with novel coronavirus. Japan, South Korea and Thailand also report their first cases one day prior.
China imposes aggressive containment measures in Wuhan, the epicenter of the outbreak, suspending flights and trains and shutting down subways, buses and ferries in an attempt to stem the spread of the virus.

Improve Distribution Of Resources And Information For Vulnerable Populations
Many people live in slums or overcrowded settlements with little access to health services or to soap, water, and sanitation. There are also areas of high population density where many households share a single room, making physical distancing nearly impossible. Some communities have received little information about COVID-19. The lack of outreach could have led to fear of seeking testing and treatment services due to stigma and discrimination. More attention should be paid to vulnerable populations. The government should recruit and train more health workers to reach out to vulnerable communities, and regularly distribute personal protective equipment and provide sanitation services to them.

On January 27, the results were released of a commercially available respiratory pathogen multiplex real-time PCR for detection of pathogen genes on the NPS/OPS samples at the RITM Molecular Biology Laboratory. These assays reported detection of Influenza B viral RNA, human coronavirus 229E viral RNA, Staphylococcus aureus DNA and Klebsiella pneumoniae DNA. A 10-day course of oseltamivir 75 mg BID was given on the basis of the influenza result. The NPS/OPS specimen was then sent by RITM to the Victorian Infectious Disease Reference Laboratory in Melbourne, Australia, for COVID-19 testing .
On January 29, further NPS/ORS specimens were collected and sent to the RITM. On January 30, the result of the initial NPS/OPS sent to VIDRL reported detection of 2019-nCoV viral RNA by real-time PCR. The patient was thus identified by the Department of Health as the first confirmed COVID-19 case in the Philippines .
Why Case Numbers Could Track Upwards
A two-week time-lag from when Auckland shifted to a softer lockdown a fortnight ago also meant that any resulting effect on case numbers could become apparent over coming days.
“One scenario is that we do start to see a trend, where the numbers will bounce around but clearly track upwards,” Covid-19 modeller Professor Michael Plank said.
“The other is that they hover somewhere in the ballpark of the teens or 20s, which will indicate contact tracing teams are still able to keep a lid on things at level 3.”
If numbers did start to grow, Plank said they might keep rising gradually, or sharply, as seen in Australia.
“Victoria had quite a rapid increase in cases from 20 to around 1500 in six weeks – all with similar vaccination rates, and restrictions similar to level 3.”
Another expert, University of Auckland computational biologist Dr David Welch, said a continuing rise in cases particularly unlinked ones already made it clear that level 3 wasn’t working to quash the outbreak.
“Modelling suggested that the move from level 4 to level 3 was a risk – we have not landed on the right side of that risk.”
Eliminating Covid-19 in the short term, he added, would require a move back to level 4 something Prime Minister Jacinda Ardern emphasised wouldn’t be considered by Cabinet today.
She also pointed out that there was no evidence to suggest that the shift to level 3 had brought about a change in case numbers.

Censorship And Police Responses
A pneumonia cluster of unknown cause was observed on 26 December and treated by the doctor Zhang Jixian in Hubei Provincial Hospital, who informed the Wuhan Jianghan CDC on 27 December. The early response by city authorities was accused of prioritising a control of information on the outbreak. A group of eight medical personnel, including , an ophthalmologist from Wuhan Central Hospital, who in late December posted warnings on a new coronavirus strain akin to SARS, were warned by Wuhan police for “spreading rumours” for likening it to SARS.
By the time China had informed the of the new coronavirus on 31 December 2019, commented that the government was still keeping its own citizens in the dark in an opinion published on . While by a number of measures, China’s initial handling of the crisis was an improvement in relation to the SARS response in 2003, local officials in Wuhan covered up and downplayed the initial discovery and severity of this outbreak. This has been attributed to the censorship institutional structure of the country’s press and Internet, with Jude Blanchette of the quoted stating “under Xi Jinping, the inclination to suppress has become endemic and, in this case, contributed to a prolonged period of inaction that allowed the virus to spread”. William Summers, a professor of medicine, told though that such silencing and downplaying tactics are not unique to China, and seems to be standard operating procedure worldwide.
