  • If your vaccine record has missing or incorrect information, consult the tips on our Covid Records Helpdesk.
  • You will need to get a new vaccine record every time you get another dose.
  • Vaccinated in Alberta without personal health number

    • If you were vaccinated in Alberta but don’t have an Alberta Personal Health Number, call 811 to get a vaccine record with QR code mailed to you.
    • If you become eligible for an Alberta Personal Health Number, you can use it to access your vaccine record online at

    Alberta residents vaccinated outside Alberta

    • If you or your children under 18 were partially or fully vaccinated outside of Alberta, update your health records by submitting your digital out-of-province/country immunization records online at
    • Submitted records will be reviewed by AHS:
    • If more information is required to validate your record, AHS will contact you.
    • If your record cannot be validated, a letter will be sent. Book an appointment to begin or complete a valid COVID-19 vaccination series.

    Key To Nyc Vaccine Requirement Enforcement

    Enforcement of the citys Key to NYC vaccine requirement for all workers and patrons of New York businesses began Monday, Sept. 13.

    Capturing the moment that the plane touched down and history was made, as the first 600,000 doses of COVAX vaccines arrived in Africa this morning at Kotoka International Airport, in Accra. The first of at least 2 billion doses to be supplied by COVAX in 2021, these doses of the Oxford-AstraZeneca vaccine were flown in overnight from India, where they were manufactured by the Serum Institute of India.

    The precious cargo was quickly loaded into refrigerated trucks and transported to a vaccine storage facility in preparation for the first vaccinations of health care workers in the coming days.

    Yesterday at Serum Institute of India, final preparations were underway for the first shipments of COVAX vaccines ready for transportation to Africa. The first 600,000 doses set off, bound for Ghana.

    De Blasio Announces Vaccine Mandate For All Nyc City Workers

    New York City Mayor Bill de Blasio on Oct. 20 announced a new COVID vaccine mandate for all municipal workers, including police officers and firefighters.

    The citys previous vaccine requirement only applied to Department of Education staff and health care workers at NYC Health + Hospitals, but the new mandate would mean all city workers would need to get their first shot by Nov. 1.

    Between Oct. 20 and Oct. 29, city employees will receive an extra $500 in their paycheck for receiving their first shot at city-run vaccine sites, the mayor said.

    Unvaccinated employees will be placed on unpaid leave until they show proof of vaccination to their supervisor.

    Medical Contraindications To Covid

    Medical contraindications against COVID-19 vaccines are limited in number, and include:

    • a history of severe allergic reaction after previous administration of a COVID-19 vaccine using a similar platform
    • an allergy to any component of the specific COVID-19 vaccine or its container
    • a history of major venous and/or arterial thrombosis with thrombocytopenia following vaccination with AstraZeneca COVID-19 vaccine
    • a history of capillary leak syndrome following vaccination with AstraZeneca COVID-19 vaccine

    As a precaution, the National Advisory Committee on Immunization recommends that people who experienced myocarditis and/or pericarditis after a first dose of an mRNA COVID-19 vaccine should wait to get their second dose of mRNA vaccine until more information is available.

    If you have a history of myocarditis or pericarditis and are still being followed by a doctor for related heart issues, please follow up with your healthcare provider before receiving the vaccine.

    Bring your CDC COVID-19 Vaccination Record card with you to your appointment if you need another shot of COVID-19 vaccine so your provider can fill in the information about your additional shot.

    If you have lost your CDC COVID-19 Vaccination Record card or dont have a copy, contact your vaccination provider directly to access your vaccination record.

    • If you cannot contact your vaccination provider site directly, contact your state health departments immunization information system . Vaccination providers are required to report COVID-19 vaccinations to their IIS and related systems.
    • If you enrolled in v-safe or VaxText after your first vaccine dose, you can access your vaccination information using those tools.
    • If you have made every effort to locate your vaccination information, are unable to get a copy or replacement of your vaccination card, and need another COVID-19 vaccine dose, talk to a vaccination provider.

    CDC does not maintain vaccination records or determine how vaccination records are used, and CDC does not provide the white CDC-labeled COVID-19 Vaccination Record card to people. These cards are distributed to vaccination providers by state health departments.
