The Delta variant is different from previous SARS-CoV-2 variants that have made their way to the US. Its highly contagious and may cause more severe disease. It is also more likely than previous variants to be spread by vaccinated people experiencing breakthrough infections.
Evidence presented to the CDC showed the Delta variant to be much more contagious than previous variants, with a risk of transmission similar to chickenpox. It also appears that people who are infected can spread the virus for longer periods of time.

ISTANBUL — A Turkish study of more than 30 million vaccinated citizens indicated that the highest level of protection was seen in people who received three doses of inactive vaccines, rather than in those who received two doses of inactive vaccines and one booster shot of an mRNA vaccine, the country’s Health Minister Fahrettin Koca said on Wednesday.
While the minister avoided specifying brand names, he was referring to the vaccines now available in Turkey: the Chinese inactive vaccine CoronaVac by Sinovac Biotech and the mRNA offering developed by Pfizer and BioNTech.
While many Turks have preferred receiving a Pfizer booster shot, believing it is more effective than the Chinese vaccine, the research shows that using the same vaccine type works better. Health authorities in Turkey and other countries had been recommending that people inoculated with two doses of inactive vaccines receive an mRNA jab as a third dose to boost protection.
“Having three doses in designated groups is very important,” Koca said. “People around the world will learn this data for the first time, and countries’ vaccination policies will perhaps change.”
He added that the researchers who carried out the study on Wednesday applied to a scientific journal to publish the full results.

Fully vaccinated people had 6-13X the odds of infection compared to previously infected but unvaccinated people, depending on the model used. The risk for previously infected patients with one dose of the vaccine was the lowest of all.
What Are Breakthrough Infections And Why Do They Happen
A breakthrough infection is one that occurs after full vaccination.
In an article published in MMWR, the CDC reported 10,262 breakthrough infections through the end of April 2021. By that point, more than 100 million Americans had received the COVID-19 vaccine.
The vast majority of breakthrough infections were asymptomatic, mild, or moderate. About 1,000 people with breakthrough infections were hospitalized, and 160 died, though the hospitalizations and deaths were not always related to COVID-19.
These numbers tell us that the vaccines are doing a good job preventing infection and severe illness. None of the vaccines were 100% effective in clinical trials, so a small number of breakthrough infections was expected.

New research suggests that people who have already gotten sick with COVID-19 benefit from the vaccine, possibly even more than vaccinated people who were not previously infected.
A study published in NEJM found that people who were previously infected with SARS-CoV-2 and then received a single dose of either mRNA vaccine experienced a rapid immune response and generated at least as many, or more, antibodies as previously uninfected people who had received two doses of vaccine.
And a study published in Science compared the effects of the Pfizer vaccine in people with and without prior infection. The researchers found that previously infected people who were vaccinated had a stronger immune response against the B.1.1.7 and B.1.351 variants than previously uninfected people who were vaccinated.

In August 2021, the FDA authorized a third dose of an mRNA vaccine for certain people who are immunocompromised. The third dose should be given at least 28 days after the second dose.
The COVID-19 vaccines, like other vaccines, work by stimulating a persons immune system to produce antibodies against the virus. If the antibodies later encounter the actual virus, they are ready to recognize and destroy it.
A person with a weakened immune system may not be able to produce a strong enough immune response to the standard, two-dose COVID mRNA vaccine regimen. This leaves them more likely to have a breakthrough infection, and increases the risk of more severe disease if they do become infected. A third vaccine dose may enhance the immune response in some people who are immunocompromised.
The FDA authorized a third dose for people who are moderately to severely immunocompromised. A person may have a weakened immune system for a variety of reasons. These include being born with an immunodeficiency, having advanced or untreated HIV, and having a chronic medical condition such as chronic kidney disease or lack of a spleen. Many types of medication also suppress the immune response, including some cancer treatments, TNF blockers, some biologic agents, and high-dose corticosteroids. Low Vaccine Uptake Driving Up Infections
The unvaccinated population is at high risk for infection. If this variant continues to move quickly, especially in areas of low vaccination rates, the U.S. could see a surge in SARS-CoV-2 infection, said Dr. Miriam Smith, chief of infectious disease at Long Island Jewish Forest Hills Teaching Hospital in Queens, New York City.
CDC Director Dr. Rochelle Walensky issued a warning on this potential surge earlier this month.
In a press briefing, she said preliminary data suggested that 99.5 percent of the people who died from COVID-19 since January were unvaccinated.
We know that the Delta variant is currently surging in pockets of the country with low vaccination rates, she said.
In Missouri, which has a vaccination rate of nearly 43 percent, confirmed coronavirus infections surged in July.
In contrast, Vermont reported only 94 cases for the past day and currently has one of the highest vaccination rates in the country at 69 percent.
This echoes findings from a U.K. study that found the Delta variant twice as likely to lead to hospitalization, and both the AstraZeneca-Oxford and Pfizer-BioNTech COVID-19 vaccines being effective in reducing this risk.
We also know that our authorized vaccines prevent severe disease, hospitalization, and death from the Delta variant, Walensky said.
Dr. Theodore Strange, the interim chair of medicine at Staten Island University Hospital in New York, said the data supports this.

Oxford Universitys initial mix-and-match study, Com-COV1, is focused on mixing the AstraZeneca and Pfizer jabs. Com-COV2, however, is evaluating the safety and immunogenicity of combining the Moderna and Novavax vaccines with a first dose of either the Pfizer or AstraZeneca jab.
The researchers say that as more Covid-19 vaccines become available, the number of jabs included in the vaccine mixing study may increase. Dose-mixing studies testing other vaccine combinations are also ongoing around the world, including a Moscow trial of an AZ-Sputnik V combination, as well as a Philippines-based study mixing Sinovacs CoronaVac jab with six other vaccines.
Experts Are Excited To Have A Single
The most obvious benefit of the J&J vaccine is it’s just one shot.
“It’s nice to have a single-dose vaccine,” FDA committee member Eric Rubin, editor in chief of the New England Journal of Medicine, said Friday evening after the vote. “The demand is so large that it clearly has a place.”;
The rest of the FDA committee agreed.
“What we have to keep in mind is we are still in the midst of this deadly pandemic,” committee member Archana Chatterjee, dean of the Chicago Medical School, said. “There is a shortage of vaccines that are currently authorized, and I think authorization of this vaccine will help meet the needs of the moment.”
