Why are many health care workers refusing to get the COVID-19 vaccine?

While a majority of nurses are vaccinated and more than half support vaccine mandates in the workplace, some are pushing back against requirements to get vaccinated or face mandatory testing and say they would rather leave their jobs. And hospitals are already feeling the effects.

One New York hospital announced that it would no longer be able deliver babies because dozens of staff members quit rather than get vaccinated. In Houston, 153 hospital workers resigned or were terminated over a recent vaccine mandate. It’s a crisis on top of the widespread shortage of nurses that’s been a problem since even before the pandemic.

A nurse has filmed herself supposedly being escorted from her job at a hospital because of her refusal to take the Covid-19 vaccine.

The video, posted on Twitter on Saturday, shows the woman telling the camera she is being removed from the Kaiser Permanente hospital because I dont want to get the jab.

The nurse says she is not willing to get the vaccine because of her sincerely held religious beliefs as she urges people to count the costs and says she is willing to lose everything, for my freedom.

She is seen walking through hospital corridors followed by security personnel.

The video had been viewed more than 5.4 million times as of Monday morning.

More Than 50% Of Nurses Reluctant

Skepticism is not uncommon among medical staff in Germany. A survey conducted by the “German Society for Internal Intensive Care and Emergency Medicine” and the “German Interdisciplinary Association for Intensive Care and Emergency Medicine” back in December last year showed that only 73% of doctors, and just under 50% of nurses, in Germany want to be vaccinated against SARS-CoV-2. Although a clear majority of respondents said the vaccine was important to contain the pandemic.

A spokesman for a German federation of care homes, BPA, however, paints a rather confusing picture. BPA President, Bernd Meurer, said: “We know of care facilities where almost 100% of the staff are vaccinated. In others, two-thirds have refused to do so.”

Health Minister Jens Spahn struck a similar note this week. He spoke of nursing homes where vaccination is already being carried out and where the vaccination rate among the staff has reached 80%. However, in other homes he said, the rate is only 20%.

Concerns Raised As Nurses Who Refuse Covid

The Royal College of Nursing has written to a hospital trust in the South of England to express a range of concerns about its decision to redeploy nurses who declined the Covid-19 vaccine to non-patient facing roles.

University Hospital Southampton NHS Foundation Trust has this week confirmed that a small number of staff who refused to have the Covid-19 jab would be moved away from working directly with patients.

It said that, while more than 94% of staff at the trust had chosen to be vaccinated against Covid-19, it had to take further action to live up to its commitment of a zero Covid approach.

The RCN said that nurses were among those who had been redeployed and urged affected its members to contact the union to discuss their options.

It has written to the trust to express our concerns, which include breach of contract, breach of individuals right to confidentiality and possible disability discrimination for those who cannot have the vaccine for medical reasons.

RCN regional officer for south Hampshire, Debbie Schilder, said the college were also worried about the impact redeployment would have on staffing levels.

As part of our wider infection prevention strategy, a small number of staff who have declined the vaccine will be redeployed away from patient-facing roles, it said.

Any reduction of clinical staff and their supporting staff increases the chances we will have to further reduce staffed beds, Upstate said in a prepared statement. We will monitor closely to optimize operations to safely care for the patients we serve. We continue efforts to recruit and retain staff, which includes seeking ability to competitively compensate and improve quality of work environment.

The state mandate applies to health care workers in hospitals, nursing homes, adult care facilities, home care agencies and hospices.

Another 120 New Jersey hospital workers were terminated or quitthis weekafter refusing to get vaccinated for COVID-19.

Virtua Health, a health system that operates five hospitals in South Jersey, is the latest to see more than 100 employees leave their jobs after not complying with a network vaccine mandate. Earlier this week, RWJBarnabas Health announced it had terminated 118 workers for refusing to comply with its policy.

Unfortunately, about 120 people have elected to discontinue their employment due to the vaccination policy, Dennis W. Pullin, president and CEO of Virtua Health, said in an emailed statement. While that is their right, we had hoped they would have chosen to receive this very safe and effective vaccine in order to protect themselves, our patients, and others. Vaccines are our best hope for reducing serious illness, long-term health problems, and loss of life from COVID-19.

Pullin noted that about 98% of the health systems workforce is vaccinated and those who received medical or religious exemptions will participate in routine COVID testing.

Do They Know Something We Dont

The AMA Code of Medical Ethics states that: Informed consent to medical treatment is fundamental in both ethics and law. Patients have the right to receive information and ask questions about recommended treatments so that they can make well-considered decisions about care.

1 in 8 doctors and nurses hesitating to get vaccinated raises the question whether they know something the rest of us dont. And if so, can we say with confidence that current risk disclosures create informed consent?

Disclosure for the Pfizer vaccine have no doubt been parsed by doctors and lawyers within and without the company and government. In one place, the disclosure says, There is a remote chance that the Pfizer-BioNTech COVID-19 Vaccine could cause a severe allergic reaction.

Do the words remote and severe inform or obscure? Remote seems to connote, too small to worry about, while severe represents an adjective so squishy that it overwhelms the noun it modifies.

Is the disclosure therefore a loaded phrase? Would we say, for example, that for otherwise healthy people under the age of 30, there is a remote chance that catching COVID will result in severe illness?

Pfizer has hard numbers from its clinical trials. Would its disclosures be more informative if they recited these numbers?

Many Doctors And Nurses Have Antibodies

A nurse at a hospital in Brandenburg, who spoke to DW under the condition of anonymity, said he was skeptical of the BioNTech-Pfizer vaccine, of which he had already received the first dose. “It does not prevent infection with the coronavirus. It only prevents the actual outbreak of the COVID-19 disease. So I can imagine that others prefer to wait for other vaccines.” Many doctors and nurses, he adds, have already been infected with the virus, recovered, and have formed antibodies. And therefore do not need the vaccine.

But in his work environment, he says all colleagues would get vaccinated. “We have seen too often in the last few months what the virus can do.”

Meanwhile, the overall willingness to be vaccinated has increased in Germany. The monthly Deutschlandtrend poll records 54% of those polled as saying they want to be vaccinated. That is a marked rise compared to last November.

Added to this are 21%, who say they’d “probably” want to receive the vaccine. German scientists have said over 60% of the population needs to be vaccined to achieve the so-called herd immunity. This, in turn, would be enough to contain the pandemic.

Prior to introducing a mandate, there needs to be adequate supply and equitable access to vaccines. We need to ensure people have the opportunity to review vaccine safety and effectiveness data and to get the vaccine of their free will.

Careful planning, consultation and communication with key groups can improve acceptability of mandates.

In the coming weeks, more health workers are likely to resign or be dismissed for failing to comply with the COVID mandates. There will be those in social media who will call out the situation as the right move. But some health workers will become privately or publicly vocal on the issue and will cast doubt on the vaccine. It is important we prepare for these situations, especially in regional areas where there may be fewer voices and greater trust in long-serving health workers.

Why Do Us Health Care Workers Continue To Refuse Vaccination Against Covid

In January, when COVID-19 vaccines were first rolled out among US health care workers, news stories quickly surfaced with reports that large numbers of health care workers were refusing to be vaccinated.

In a press briefing on January 4, Ohio Governor Mike DeWine announced that roughly 60 percent of the states nursing home workers refused the first round of vaccinations.

Other areas reported a similar phenomenon. In Los Angeles County, between 20 and 40 percent of all health care workers refused the vaccine when first available, and 50 percent in neighboring Riverside County refused.

Dr. Jeremy Boal, the chief clinical officer at the Mount Sinai Hospital system in NYC, told the Gothamist in early January that across the eight hospitals in the system, vaccine acceptance ranged from 25 to 65 percent.

The trend of vaccine skepticism among health care workers is especially dangerous because these workers are in more frequent contact with both COVID-19 patients and patients who are more vulnerable to deadly COVID-19 complications. Health care workers also play a crucial role in influencing the general population to accept the vaccine.

They Work In A Culture Of Intimidation And Abuse

Speaking out means risking ones job, medical license, nursing license, reputation, future. Deborah Conrad, a physicians assistant in New York, can attest to this. When she started speaking out about patient complaints, vaccine side effects and rampant medical malpractice to hospital administrators, she was met with rebuke. When she filled out a report with the Vaccine Adverse Events Reporting System on behalf of a vaccine injured patient, the hospital administrators scolded her for spreading vaccine hesitancy. She is obligated by law to file the report, yet her administrators wanted her to conceal this information from the reporting system. She has filed 130 reports with VAERS in 2021, and said she could have filed about 175 reports. Despite her concerns, her superiors refuse to answer any of her questions or listen. Deborah Conrad was just told to follow orders. All hospitals should have a system in place for healthcare professionals of all ranks to report medical malpractice, coercion and medical error. A system of accountability and transparency should be honored and protected.

Butler says her concern for getting nurses across the country vaccinated is that she wants them to be safe and protected. The best way for health care leaders to get their colleagues inoculated against COVID-19, she says, is to operate from a place of compassion.

They need workplaces that “speak to individuals in a compassionate and kind manner,” she says. “Knowing that the person talking to you wants you to get vaccinated not because it’s the law, but because they care.”

How To Tackle Vaccine Hesitancy

Health systems, medical facilities, and state officials have tried different ways to encourage more healthcare workers to get vaccinated.

Some employers have announced incentives, including cash bonuses, gift cards, raffle tickets, and even free breakfasts at Waffle House.

Others especially those in sectors that have been changed radically by the pandemic have discussed making vaccines mandatory in their workplaces.

Vaccine mandates are not unheard of. According to Johns Hopkins Medicine, 15 states have laws that require healthcare workers to be vaccinated in certain circumstances to keep high-risk patients safe. Most hospitals have made the flu vaccine mandatory.

But mandating coronavirus vaccines is more complicated, as the Food and Drug Administration has authorized them for emergency use rather than giving them full approval, so they are still considered experimental.

Benjamin worries that mandating vaccines could backfire.

“One of the things that I’ve learned in all my years of practicing health and medicine is that the minute you tell people they have to do something, then people won’t do it,” he said.

Benjamin said he preferred using the “power of persuasion,” putting out “good information” through “trusted messengers.”

This is what Butler, the founder of Nurses Who Vaccinate, has done so successfully.

“We’re really trying to get ahead of the misinformation. We want people to hear it from us first,” Butler said.

When Employers Can Mandate Employee Vaccination

We seem to be moving towards mandated vaccinations. Generally, employers may require vaccinations which are job-related and consistent with business necessity, assuming exceptions exist for religious and medical reasons.

Increasingly, not just employment but access to education, and freedom to engage in everyday commerce may require COVID vaccination. And short of mandates, onerous testing or other requirements may achieve a similar result.

An open question is how many healthcare workers will quit rather than submit. Follow-on questions include how much further trust will erode among those who submit, and how such erosion will impact our healthcare system.

Despite being prioritized recipients of the COVID-19 vaccines, many healthcare workers have refused to take the vaccine.

An early 2021 study by the Kaiser Family Foundation found that almost one-third of healthcare workers will probably or definitely refuse the vaccine. Though the American Medical Association reported in June 2021 that 96% of surveyed physicians had received the vaccine, other healthcare workers were refusing to get it for a number of reasons.

A large factor was an overall distrust of the medical system from minority groups.

Dr. Nikhila Juvvadi, the chief clinical officer at Chicagos Loretto Hospital, told NPR much of the skepticism in her hospital is due to this lack of trust from minorities.

Ive heard Tuskegee more times than I can count in the past monthand, you know, its a valid, valid concern, Juvvadi said.

The Tuskegee experiment to which she refers began in 1932 as the Public Health Service studied syphilis in Black men. As noted by the CDC, The study was conducted without the benefit of patients informed consent. Researchers told the men they were being treated for bad blood, a local term used to describe several ailments, including syphilis, anemia, and fatigue. In truth, they did not receive the proper treatment needed to cure their illness. That study lasted for more than 40 years.

Many health care workers turned down their COVID vaccine. Here’s why

While some healthcare workers do hold these extreme views, others are hesitant about getting a vaccination so soon into the rollout.

Dr. Stephen Noble, a heart surgeon in Olympia, Washington, is not generally opposed to vaccines.

“I’m not a vaccine denier or anti-vaxxer,” he told Insider. “I’ve vaccinated my kids, and I get vaccinated with the flu vaccine every year.”

But Noble said he believed the COVID-19 vaccine rollout started too quickly to instill confidence.

“One of the issues that concern me is that we just don’t have long-term data,” he said. “The vaccine was supposed to be a three-year study, but somehow, we stopped it at six months and said, ‘You know what, this data is good.'”

While Noble is declining the chance to be vaccinated now, he is not ruling it out for the future. He said he was considering getting a shot in the early summer once more data was available.

Butler, the founder of Nurses Who Vaccinate, said that she believed Noble’s views were widely shared and that many healthcare workers would eventually come around to the vaccines.

“I know some nurses have a timeline in their head,” she said. “They’re waiting until spring.”

While it is true that the vaccine has been rolled out at an unprecedented pace, experts are seeking to reassure people that corners have not been cut.

“We know, historically, most of the bad stuff that happens with vaccines occurs in the first few months,” he said. “We’re outside that window now.”
